Participant Waiver

I acknowledge that participation in the AIM wellness program carries risk of injury. 

I accept those risks of injury as solely my own and I, on behalf of myself, my personal representatives, heirs and assigns,  hereby hold AIM, its employees, members and agents, harmless from and against any and all claims, suits or causes of action of any kind for direct or consequential damages arising from any injury, whether physical or emotional, that may result from my participation in the AIM wellness program, even if arising from the negligence of the releasees or others. I willingly assume full responsibility for the risks that I am exposing myself to and accept full responsibility for any injury or death that may result from participation in any activity or class.

I acknowledge that I have no physical impairments, injuries, or illnesses that would endanger me or preclude safe participation in this program.

I understand that consultation with a medical doctor is recommended before engaging in strenuous physical activity, especially if this activity is new to me.
